If the first game is any indication, you should not start your journey with this one unless you like suffering. While it is far from being impossible, it is not something a lot of people will enjoy. Another important thing to keep in mind is that if you choose this difficulty, you will have to stick to it until the very end — it cannot be changed mid-game.
