One of the achievements in Cup: The Delicious Last Course is Hearty which requires you to have 9 HP. As complicated as it sounds, it is simple to do so albeit a little hidden. Cuphead 9 HP
In order to get 9 HP in the game, you need to choose select the Charm named Twin Heart. Once selected, you just need to run in circles around yourself for a few seconds. After a while, you will get Genie informing that you have got Extra Hearts – a total of 9. If it does not happen, try to do so in the Finale World, Inkwell Isle III.
Once that happens, you will get 9 HP for all the bosses in Easy/Normal mode. Aside from that, you will also get the Hearty achievement which is needed to 100% the game. Having 9 HP also makes some of the bosses a pushover … unless you are as bad as I tend to be.

Once you get the hearty upgrade, fight king dice on regular with the twin heart equipped and land on two heart spots to get the achievement. It’s very obscure but that’s what you do
