By collecting all gems you can unlock one type of skin. To get gems you must find crates and break them open or finish levels without dying more than three times. You need to be extremely focused and memorize each level to clear them without dying three times. There are a ton of other challenges on each level for you to complete such as the Time Trials and the N.Verted variants of stages. You can complete them for additional gems.
