To recruit Us, you need to start the Escape the Nautiloid mission. As you search the ship’s chambers, you will meet Myrnath, a wounded elf. You need to interact with Myrnath to trigger a dialogue with Us, the Intellect Devourer. To successfully recruit Us, select one of these actions:

- Gently free the brain from its enclosure — Dexterity check
Attack on Moonrise Towers
After completing the Prologue, you will reunite with Us near the start of Act-3 during the Attack on Moonrise Towers. While confronting Ketheric Thorm on top of the tower, you will be able to drop into a hole.
Once you are in the Mindflayer Colony, continue a little forward into the Colony’s Morgue. In here, you will meet Chop, a Bugbear. You can persuade Chop to release Us. Alternatively, you can also fight to get a key that frees him.
